It is refreshing to see that more and more women are educating themselves on the importance, safety and efficacy of Hormone Replacement Therapy (HRT). Many women however are unaware of the adverse and beneficial differences between “synthetic” and the “bio-identical” forms of HRT.

Synthetic forms such as Premarin and Prempro are manufactured from horse urine and are not exact replicas of innate natural hormones like the Bio-identicals are. Because of this, the body does not utilize or metabolize them in the same way it does its own biological hormones thus potentiating the adverse side effects seen in the WHI study in 2002 such as increased cancer risk, heart attack, stroke and death.

So what can women do when they experience hot flashes, mood swings, disturbed sleep, diminished/absent sex drive, memory disorder, sleep disturbance, weight gain and the other dozen or so symptoms commonly experienced in menopause  They are afraid of the consequences of the synthetic prescription hormones that her doctors are suggesting and are confused as to the reality regarding the safety and efficacy (positive results) of Bio-Identical Hormone Replacement Therapy (BHRT). The bio-identical forms of HRT have been around for over 75 years but have lost popularity since the profit making patentable forms of synthetic hormones were introduced.

A recent literature review of 20 studies performed on Bio-Identical forms of HRT showed significant improvement in menopausal symptoms when compared to placebo and the “synthetics”. It also showed a definitive decrease in the incidence of heart attack, stroke, cancer, Alzheimer’s, Parkinson’s and osteoporosis that in the past was thought to occur with the synthetics as well but was proven the contrary according to the 2002 WHI study.

Just as cheese comes in different forms; block, grated, sliced, aerosolized, canned etc., so do bio-identical hormones. Each of these different vehicles has their own distinct advantages and disadvantages but all are tailored made to fit each individual patient’s needs and likes. Bio-identical hormones are manufactured by a compounding pharmacist from soy or yam and are available as an oral route in the form of capsules, or a sublingual (under the tongue) drop. Other modes of delivery are intra-muscular injection, topical creams, lotions, gels and patches-(the most preferred route by many physicians – but I personally rarely prescribe these), suppository forms via the rectum and vagina and lastly, my preferred choice, subcutaneous administration. This involves the implantation of small pellets just beneath the skin through a small surgical incision. It is a painless in-office procedure with effects lasting from three to four months. The subcutaneous route of administration avoids the “roller coaster” effect of waxing and waning levels of hormones seen with all other forms of BHRT. It is safe and the most convenient and effective form of hormone administration as well as the first used in America nearly ¾ of a century ago.

The take home message is: do your homework, get a second or third opinion and don’t ignore your body’s cries for Hormonal Health. The decision is no longer should I or shouldn’t I take hormones but rather, “what delivery method of BHRT is right for me”!
